Patient Referral Promotion
The referral of friends, coworkers and family to our practice is the greatest compliment our patients can give and is the highest expression of your confidence in the personalized care we provide.
As a way of thanking you, when referring a patient to our practice, you will receive a $25 VISA gift card or credit applied to account. For a referral of 2 members of the same household, you will receive a $50 VISA gift card or credit applied to account. For a referral of 3 or more members of the same household you will receive a $75 VISA gift card or credit applied to account.
THE FOLLOWING CRITERIA MUST BE MET FOR YOU TO BE ELIGIBLE TO RECEIVE YOUR GIFT CARD:
- You must be an existing patient of record.
- The person you refer may not have been a previous patient of record.
- Your referral must complete a cleaning or other dental procedure.
- Upon meeting this criteria, you can expect our office to contact you regarding receipt of your gift card. These cards have no cash surrender value. Once an account credit or gift card is issued, it is not exchangeable or refundable.
